Invasive aspergillosis of cavernous sinus

Abstract
A 72-year-old woman was presented with progressive visual loss and headache for two months. Her visual acuity of right side was no perception of light. Brain MRI showed bulging enhancing mass in right orbit apex extending to cavernous sinus and sinusitis of sphenoid and ethmoid sinus. The patient was suffered from severe eyeball pain and headache not controlled by analgesics. An open biopsy via the extradural cavernous sinus approach was performed. Pus discharge was observed from the cavernous sinus and orbital apex under the anterior clinoid process. Histologic examination demonstrated hyphae. Systemic antifungal treatment such as Amphotericin-B and voriconalzole relived the pain but visual loss of right eye was not improved. We have to consider the fungal infection such aspergillosis as a differential diagnosis of a mass in the cavernous sinus.
